What is the Be Well Plan

The Be Well Plan is direct primary care. Direct Primary Care (DPC) is an innovative alternative payment model improving access to high functioning healthcare with a simple, flat, affordable membership fee.  We aim to remove the middleman like insurance. No fee-for-service payments.  No third party billing.  The defining element of DPC is an enduring and trusting relationship between a patient and his or her primary care provider. Our same providers that you have come to know and love, will be there throughout the year to help you and your family, be the healthiest you can be. Why did we create the Be Well Plan

At Mom’s Choice Mobile Healthcare, we want to offer a more comprehensive, more affordable approach to healthcare. We believe that healthcare should be accessible and convenient and should not cost an excessive amount. The current healthcare system is broken. It has become a war between health insurance companies and providers, with the patient stuck in the middle. Sadly, the system in our country actually hinders health rather than enhances it. Mom’s Choice Mobile Healthcare does not want to be a part of the broken system, but create a different, more sustainable model of healthcare.

Who is it for?

Our Be well Plan is for patients from birth through 65 years of age, who are relatively healthy. If you or your family members have complex health needs, it is recommended to continue seeing your specialist provider.  At this time, we do not have a Geriatric Specialist Patients over 65 have complex healthcare needs and we recommend being followed by a Geriatric Care Specialist. st.

How does it work?

Sign up is easy! Click “Request an Appointment” and this will take you to our booking site with all of the products that we provide. You will then click on the Adult or Youth Be Well Plan. Be sure to add all family members you wish to enroll. You will be charged monthly, a credit card is required to maintain monthly membership. A Be Well Plan agreement will be sent via email upon enrollment.


Do we accept insurance?

With Direct Primary Care, insurance is not accepted

How long does the Be Well Plan last

We ask that you sign up for a minimum of 1 year. This allows us to set attainable health goals. We will then consult throughout the year to ensure your goals are being met.  If at any time you become unsatisfied with our service, you the right to cancel.  Please note, a 60 day notice is required. Upon cancellation, you will be reverted to our basic urgent care service.


What does it include?

The Be Well Plan includes the following:

  • A Dedicated Provider
  • Exclusive access to your provider via the app*
  • Each family will receive a welcome kit with a thermometer, otoscope, pulse ox, Blood Pressure cuff, and scale.
  • 1 comprehensive physical exam**
  • Common childhood and adult vaccines**
  • Unlimited sick house calls *
  • Unlimited televisits, from anywhere in the United States*
  •  Testing that Provider deems to be necessary
  • One IV therapy yearly for patients 18 years and older, who are enrolled in the plan (may not accumulate from month to month).

Can I Cancel at any time?

Cancellation is easy, simply send an email to info@momschoicehealthcare.com indicating you would like to cancel. We require a 60 day cancellation notice. There will be no refunds, or prorated refunds,  for months that are unused.

What is the Fee?

There is a $200 sign up fee per Family at the beginning of each year. Adult Be Well Plans will be charged monthly at a rate of $300 and the Youth Be Well Plan will be charged a monthly rate of $150 per month. There are no extra charges for house call visit or telemedicines visits.
