What is the Be Well Plan?

The Be Well Plan is direct primary care. Direct Primary Care (DPC) is an innovative alternative payment model improving access to high functioning healthcare with a simple, flat, affordable membership fee. It acts to replace traditional healthcare plans. No more middleman! Sayonara fee-for-service payments! Our same providers that you have come to know and love, will be there throughout the year to help you and your family, be the healthiest you can be. Why did we create the Be Well Plan?

At Mom’s Choice Mobile Healthcare, we want to offer a more comprehensive, more affordable approach to healthcare. We believe that healthcare should be accessible and convenient and should not cost an excessive amount. Our broken healthcare system has become a war between health insurance companies and providers, with the patient stuck in the middle. Sadly, our current healthcare system hinders health rather than enhances it. We do not want to be a part of the broken system, rather, create a new, more sustainable model of healthcare.

Who is it for?

Our Be well Plan is for patients who want a higher level of care and want more from a primary care provider. Ideally, your health needs are straightforward. If there are complex health needs, please continue seeing your specialist provider.

How does it work?

Sign up is easy! Click “Request an Appointment” and this will take you to our booking site with all of the products that we provide. Next, click on the desired plan. Be sure to add all family members you wish to enroll. A credit card is required for enrollment and you will be charged monthly. Once you enroll, an agreement will be emailed, outlining fees and payment schedule.

Do we accept insurance?

With Direct Primary Care, insurance is not accepted. Costs are kept low by billing the patient directly. Above all, the savings are passed on to you. In addition, a detailed receipt can be provided to you in order to request reimbursement from your insurance plan. Although insurance is not accepted,  we do recommend having a form of catastrophic insurance plans in case of an emergency. The Be Well Plans cover basic primary care and most urgent care issues.

How Long Do I have to Commit?

We ask that you sign up for a minimum of 1 year. This allows us to set attainable health goals. Your provider will monitor progress throughout the year. If at any time you become unsatisfied with our service, you have the right to cancel, with a 60-day notice.  Simply send an email to info@momschoicehealthcare.com.

What does it include?

Our plans include:

  • A Dedicated Provider with extended access
  • Exclusive access to your provider via the app*
  • Each family will receive a welcome kit with a thermometer, otoscope, pulse ox, Blood Pressure cuff, and scale.
  • 1 comprehensive physical exam per year**
  • Common Adult and Child Vaccines
  • Unlimited sick house calls *
  • Unlimited televisits, from anywhere in the United States*
  •  Testing that Provider deems to be necessary
  •  IV therapy (once per year per adult enrollee)

Can I Cancel at any time?

Cancellation is easy, simply send an email to info@momschoicehealthcare.com indicating you would like to cancel. We require a 90 day cancellation notice. Refunds will not be provided.

What is the Fee?

There is a $299 sign up fee per Family at the beginning of each year. The Adult Be Well Plans are $275 per month per enrollee and the Youth Be Well Plan are $175 per month per enrollee. A 10% discount applies to families.
